Applicant’s and Employers Document Checklist for Poland Work Visa

Passport: Original Passport with validity of minimum six months and minimum two blank pages for visa stamp.

a) Two copies of first and last page of the Passport

South Korea Visa Application Form : One visa application form to be duly filled and signed by the applicant.

Photo Specification: Two recent passport size coloured photographs with matt or semi matt finish, 60%-80% face coverage, white background and without border (Size: 35mm x 45mm)

Please note: Photograph should not be more than 3 months old,scanned/stapled and should not be used in any of the previous visas

Covering Letter : Covering-letter from applicant on company’s letter head stating his name, designation,passport number, purpose,duration and seal of the company, duly signed by authorized signatory.

Letter of recommendation of employment issued by the head of the relevant central government agency (an entertainer needs a letter of performance recommendation issued by the image stuff classification committee), and official letter from the relevant government agency or documents proving the necessity of employment.

Work Permit to be arranged and sent by the South Korean Company.

a) Offer of employment or Contractor job in South Korean Company.

a) Income Tax Return for the last 3 yrs

b) Bank statement for last 6 months including address and telephone number of the bank, A/C holder’s name, address with stamp & signature of bank.

c) Salary Slips for last 3 months duly attested as true copy by the company with seal & signature.
